About the Role

The successful candidate will be responsible for:

  • Supporting the practice with medication reviews, queries, and prescribing safety
  • Leading the practice medicine monitoring reviews and supporting our pharmacy technician and prescribing clerk
  • Working with the multidisciplinary team to improve medicines management and safety within the practice
  • Providing a point of contact for the practice for all medicines-related queries for healthcare professionals and patients
  • Overseeing the MHRA alerts/CAS alerts, to ensure they are audited, actioned, and logged on a spreadsheet
Key Responsibilities

The post holder will be responsible for:

  • Reconciliation of medicines from outpatient and discharge letters, including liaison with hospital, community, and primary care colleagues
  • Working with the pharmacy team and support GPs and other practice staff to deliver on Medicines Optimisation agenda (Prescribing Quality Scheme), QOF, and locally commissioned enhanced services
  • Encouraging, supporting, and implementing adherence to local formulary guidelines and cost-effective prescribing
  • Reviewing, issuing, and converting acute medicine requests into repeat medicines, where appropriate
  • Supporting Community Pharmacists in ensuring NOMAD or batch prescriptions are up to date, including confirming any stopped or initiated medications with the patient and community pharmacy team
  • Implementing and monitoring a practice's adherence to a repeat prescription policy
  • Supporting the Practice Pharmacy Technician (or prescription clerks) with the management of the repeat prescription processes and other medicines management-related duties
  • Contributing to repeat prescribing safety, prompting patients for reviews, blood tests, monitoring, etc.
  • Working with the multidisciplinary team to improve medicines management and safety within the practice, including updating, writing, and ensuring compliance with practice policies
  • Dealing with patients' medicine and prescription-related queries over the phone, in clinic, via letter/email, or the practice website
  • Signposting patients to appropriate services and other healthcare professionals (community pharmacists)
  • Working closely with GPs to provide medicine-related updates and helping with problem-solving
Requirements

The ideal candidate will have:

  • Personal Qualities & Attributes: Commitment to reducing health inequalities, ability to communicate effectively, and lead on medicines management
  • Professional Registration: Mandatory registration with the General Pharmaceutical Council
  • Qualifications & Training: Completion of an undergraduate degree in pharmacy, registration with the General Pharmaceutical Council, and holds or working towards an Independent prescribing course
  • Experience: Minimum of 2 years post-qualifying experience in hospital, general practice, or CCG, with experience and awareness of common acute and long-term conditions
  • Knowledge: In-depth therapeutic and clinical knowledge, understanding of evidence-based healthcare, and excellent interpersonal, influencing, and negotiating skills
