Job Summary

We are seeking an experienced Practice Nurse to join our teams at the Swingbridge and Welby Practices, based in Grantham and Bottesford. This full-time role offers a dynamic blend of responsibilities, with days dedicated to care coordination and days focused on practice nursing.

Main DutiesCare Coordination

Take on a pivotal role in managing and coordinating care for patients, ensuring smooth communication and comprehensive support. Work closely with the Integrated Neighbourhood Team and Primary Care Network to proactively engage with patients at risk of hospital admission or health deterioration.

Practice Nursing

Provide routine nursing care, including routine immunisation schedules, cervical screening, contraception reviews, and family planning. Work closely with the GP Practice environment to deliver high-quality patient care.

Person Specification
  • Good understanding of QOF, Enhanced Services, and CQC, NICE guidelines.
  • Good IT, organisational, communication, and clinical skills.
  • Experience of working within a nursing team essential.
  • Experience of working in primary care desirable.
  • Experience of working in a GP practice desirable.
