We are seeking an experienced Practice Nurse to join our teams at the Swingbridge and Welby Practices, based in Grantham and Bottesford. This full-time role offers a dynamic blend of responsibilities, with days dedicated to care coordination and days focused on practice nursing.
Main Duties
Care Coordination: Take on a pivotal role in managing and coordinating care for patients, ensuring smooth communication and comprehensive support. You will work closely with the Integrated Neighbourhood Team and Primary Care Network to proactively engage with patients at risk of hospital admission or health deterioration.
Practice Nursing: Provide routine nursing care, including routine immunisation schedules, cervical screening, contraception reviews, and family planning. You will also be responsible for chronic disease management, dementia reviews, cancer care reviews, health promotion, and disease management.
