Job Title: Community Health Nurse
Job Summary:
We are seeking an experienced Community Health Nurse to join our team at East Coast Community Healthcare CIC. As a Community Health Nurse, you will play a vital role in promoting the health and well-being of our patients and communities.
Key Responsibilities:
- Contribute to the development and implementation of clinical strategies and policies that promote high-quality patient care.
- Collaborate with multidisciplinary teams to deliver planned and unplanned services that meet the needs of our patients.
- Work with patients to promote self-care and independence, utilizing health coaching techniques to support their health and well-being.
- Contribute to the evaluation of training programs to support patients and carers in gaining the necessary knowledge and skills to maintain independence.
- Take responsibility for personal and professional development, maintaining competence, knowledge, and skills commensurate with the role.
- Ensure compliance with Health and Safety Policy, Fire and Environmental Waste Regulations, and ECCH policies and procedures.
Requirements:
- Registered Nurse with a valid nursing registration.
- Experience in community health nursing, with a strong understanding of clinical governance and risk management.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, carers, and multidisciplinary teams.
- Ability to work independently and as part of a team, with a strong commitment to delivering high-quality patient care.
