Key Responsibilities- Contribute to the development and implementation of case management programs for patients with complex needs.
- Assess and manage a caseload of patients with long-term conditions, providing tailored interventions to improve health outcomes.
- Work collaboratively with multidisciplinary teams to coordinate care and ensure seamless transitions between services.
- Develop and maintain partnerships with community-based organizations and healthcare providers to enhance patient care and support.
- Provide education and training to patients, carers, and healthcare professionals to promote health and wellbeing.
- Registered Allied Healthcare Professional or Registered Level 1 Nurse with a degree or diploma in Health or Nursing Studies.
- Experience of working in a community or in-patient setting, with a strong understanding of case management principles and practices.
- Excellent communication and interpersonal skills, with the ability to motivate and support staff.
- Ability to work under pressure and prioritize tasks effectively, with a strong commitment to delivering high-quality patient care.
