The Pharmaceutical Care Specialist plays a crucial role in enhancing the delivery of healthcare services within the community. This position focuses on the implementation of new service specifications aimed at optimizing medication use and improving patient outcomes. Responsibilities include addressing issues related to over-medication, inappropriate antibiotic use, and ensuring the withdrawal of unnecessary medications through established healthcare programs.
Key Responsibilities
The primary duties of the Pharmaceutical Care Specialist include:
- Conducting comprehensive medication reviews for patients with chronic conditions, particularly those with complex medication regimens.
- Providing expert guidance on medication use and optimizing treatment plans tailored to individual patient needs.
- Collaborating with healthcare teams to enhance patient care, reduce medication waste, and promote self-management of health.
- Engaging in network-wide initiatives to manage safety alerts and assist with quality audits.
- Participating in regular meetings to discuss prescribing practices and strategies for improvement.
Practice-Specific Duties
In addition to network responsibilities, the role includes:
- Ensuring compliance with regulatory standards and maintaining up-to-date prescribing protocols.
- Supporting care homes with annual medication reviews and discharge summary reconciliations.
- Leading medication review clinics focused on various health conditions.
- Training and supervising staff in medication management processes.
- Providing telephone consultations to address patient inquiries regarding medications and health management.
Qualifications
Essential
- A Master's degree in Pharmacy (MPharm) or an equivalent qualification.
- Registration with the General Pharmaceutical Council.
Desirable
- Independent Prescriber status and current registration with the GPhC.
Experience
Essential
- A minimum of two years of experience in a clinical setting with a strong understanding of therapeutic practices and evidence-based healthcare.
- Familiarity with primary care prescribing and concepts related to medication optimization.
