Clinical Pharmacist Specialist Palliative Care

We are seeking a highly skilled Clinical Pharmacist Specialist Palliative Care to join our team at BHRUT NHS. As a key member of our pharmacy team, you will play a vital role in providing clinical pharmacy services to patients requiring palliative care at our hospitals and hospice.
Main Duties and ResponsibilitiesThe successful candidate will be responsible for:
- Providing comprehensive, patient-oriented pharmacy advice and support services at ward level for our hospitals and hospice.
- Supporting community-facing hospice staff, particularly the Specialist Community Crisis and Support (SCCS) team.
- Providing specialist palliative care pharmaceutical advice to the hospice and home nurse/HCA team.
- Facilitating good supportive relationships with BHR/SW Essex Community pharmacy services and BHR/SW Essex pharmacy leads, also NELFT Community services pharmacy leads.
We are a dynamic and forward-thinking organisation delivering a full range of specialist palliative care services for all patients with life-limiting illness. Our hospice supports five Clinical Commissioning Groups and has a robust Governance Structure in place. We achieved 'Outstanding' in our CQC inspection in April 2016.
Person SpecificationThe ideal candidate will have:
- Postgraduate Diploma in General Pharmacy Practice/Clinical Pharmacy (or equivalent qualification).
- Comprehensive experience as a clinical pharmacist practitioner, including a solid grounding in all aspects of hospital pharmacy practice.
- Good organisational and time management skills, with the ability to meet set targets and deadlines.
