Bespoke Personnel is seeking a dedicated and experienced Care Coordination Supervisor to oversee the delivery of high-quality care services. This role involves managing a team of care/support workers and ensuring that all service users receive the best possible support tailored to their individual needs.
Key Responsibilities- Initial Engagement: Lead and participate in the initial meeting with service users to introduce care/support workers, fostering a positive relationship from the outset.
- Client Management: Maintain responsibility for a designated group of clients, establishing strong working relationships with social workers and other healthcare professionals.
- Performance Monitoring: Compile and manage client information and care actions, reporting weekly using Key Performance Indicator (KPI) forms to assess performance and compliance.
- Service Reviews: Organize and conduct regular reviews of service users, updating care/support plans and risk assessments as necessary.
- Coordination: Collaborate closely with Care Coordinators to ensure appropriate matching of service users with care/support workers.
- Coverage Management: Ensure adequate staffing coverage during periods of absence due to holidays or sickness.
- On-Call Duties: Undertake on-call responsibilities as required, documenting all activities and communications with the office.
- Training and Development: Provide field-based training to care/support workers, ensuring they operate within their competency levels.
- Quality Assurance: Work alongside the Quality and Compliance Manager to implement and uphold quality control procedures.
- Complaint Management: Escalate any complaints or safeguarding concerns to the Quality and Compliance Manager promptly.
- Collaboration: Liaise with colleagues and other care professionals to ensure a cohesive approach to care delivery.
- Assessment: Evaluate care/support workers against established standards during shadowing sessions.
- Documentation: Ensure all necessary documentation related to client assessments and reviews is completed accurately and timely.
This role is essential in maintaining the high standards of care that Bespoke Personnel is known for, ensuring that all service users receive personalized and effective support.
