Memory Assessment Nurse Specialist

This exciting opportunity is available within the Memory Assessment Service (MAS) at Nottinghamshire Healthcare NHS Foundation Trust. The successful candidate will work as part of a multi-disciplinary team to provide assessment, potential diagnosis, and signposting to other services and support networks for individuals aged 65 and over referred to the service with cognitive impairment.
About the RoleThe Memory Assessment Nurse will be responsible for reviewing and accepting referrals for individuals over the age of 65 with cognitive impairment, completing assessments to ascertain whether or not a dementia diagnosis is appropriate. The successful candidate will work closely with the multi-disciplinary team to explore symptoms and causes, and will refer for additional assessments as necessary.
Key Responsibilities- To work as part of the multi-disciplinary team to screen for appropriate referrals
- To undertake initial assessments and create a treatment plan
- To liaise with the MDT to explore symptoms and causes
- To refer for additional assessments
- To signpost for post-diagnostic support
The successful candidate will possess a RNMH qualification and have substantial experience of MDT working, CPD relevant to the current post, and current knowledge and understanding of older people's mental health needs and ability to demonstrate clinical competence. The ideal candidate will also have experience of assessing and prescribing for people with dementia, and be able to evidence knowledge of dementia.
