Job Title: Young Persons Support Worker

Hamilton Woods is seeking a dedicated and compassionate Young Persons Support Worker to join their team in Blackburn.

Job Summary:

The successful candidate will be responsible for managing a caseload of approximately 12 clients aged 16-25, providing support and guidance to help them achieve their goals and improve their overall well-being.

Key Responsibilities:

  • Develop and maintain strong relationships with clients, understanding their individual needs and circumstances.
  • Liaise with local authorities and other stakeholders to ensure seamless support and coordination.
  • Collaborate with colleagues to provide a comprehensive support package, addressing housing, education, employment, and personal development.
  • Monitor and evaluate client progress, identifying areas for improvement and implementing effective solutions.
  • Contribute to the development of policies and procedures, ensuring they are client-centered and effective.

Requirements:

The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment. A strong understanding of the challenges faced by young people in care or at risk of homelessness is essential.
