About the RoleThe Children and Young People's Disability Service in Camden Council offers support to children and young people with disabilities and their families through short breaks. These breaks provide opportunities for vulnerable children to achieve good outcomes in life, spend time away from their primary carer, and participate in positive activities while giving the parent/carer a much-needed break.
As a Short Breaks Practitioner, you will work with vulnerable children and young people with severe to profound disabilities and/or complex health needs to carry out high-quality and strengths-based short breaks assessments and reviews of social care packages. You will work closely with the child and their family to ensure positive outcomes and enable the child to reach their full potential.
You will facilitate and capture the views of the child/young person you work with to inform future service development and any changes in local procedures. The role requires close collaboration with a wide network of agencies, negotiation, and facilitation of support and adequate use of resources within the organisation.
You will work closely with Children's Health services, processing new referrals and deciding on the appropriate interventions of care and support.
Key Responsibilities- Carry out high-quality and strengths-based short breaks assessments and reviews of social care packages
- Work with the child and their family to ensure positive outcomes and enable the child to reach their full potential
- Facilitate and capture the views of the child/young person to inform future service development and any changes in local procedures
- Collaborate with a wide network of agencies, negotiate, and facilitate support and adequate use of resources within the organisation
