Position Overview

As a vital member of our healthcare team, the successful candidate will be entrusted with the assessment of patient care requirements and the formulation of tailored care plans. This role also encompasses the execution and appraisal of these plans, ensuring the provision of exceptional care to our patients.

In the absence of the Inpatient Unit Manager, the individual will assume leadership responsibilities to guarantee the seamless functioning of the unit.

The role includes mentoring junior staff and contributing to the education of both qualified and unqualified personnel, including students from various healthcare disciplines.

Qualifications Required

  • Registered Nurse (First Level RGN)
  • Experience in a relevant clinical setting
  • Evidence of ongoing education or a willingness to pursue a Diploma/Degree in Palliative Care; attainment of a Diploma in Palliative Care is preferred
  • Strong time management capabilities
  • Proficiency in IT or actively working towards it
  • Empathy for the mission of the organization and alignment with its core values.

St David's Hospice is a charitable organization dedicated to providing specialized care for adults facing life-limiting illnesses or requiring end-of-life support, along with their families. Our services impact over 1,000 individuals annually across various communities.

We offer round-the-clock care throughout the year, delivering clinical services alongside emotional, physical, and spiritual support. Our facilities include Day Therapy Centres that provide patient care, bereavement support, and counseling services, while our Inpatient Units focus on symptom management, respite care, and end-of-life support.

With multiple clinical sites and a Hospice at Home service, our organization is committed to ensuring that patients and their families receive the highest quality of care.

Role Responsibilities

Operational Scope:

  • Manage a 4-bed Inpatient Satellite Unit catering to:
    • Patients requiring palliative care with intricate needs, including advanced pain management and psychosocial support.
    • Coordinating safe admissions and discharges across various care environments.
    • Adjusting staffing levels based on patient needs and taking charge of the unit in the absence of Team Leaders.
    • Delegating tasks effectively among team members.
    • Assessing, implementing, and evaluating care for patients and their families.
    • Providing guidance and mentorship to junior staff and students.
    • Working flexibly to ensure a 24/7 service, including both day and night shifts.
Departmental Role
  • Collaborate within a multidisciplinary team to provide care for patients with complex needs requiring specialized palliative interventions.
  • Deliver high-quality nursing care to patients with diverse palliative care requirements, including non-cancer conditions.
  • Engage in service improvements in collaboration with management.
  • Participate in clinical governance activities, including audits and best practice implementation.
  • Serve as an educational resource in partnership with the management team and palliative care community.
Clinical Duties
  • Maintain the highest standards of care throughout the 24-hour period.
  • Coordinate safe admissions and discharges as part of a multidisciplinary team.
  • Assess, plan, implement, and evaluate care for patients and their families.
  • Foster a supportive and holistic approach to care within the team.
  • Exhibit expertise in palliative care and serve as a professional role model.
  • Adhere to organizational policies and procedures to uphold care standards.
  • Ensure accurate maintenance of patient records in line with professional standards.
  • Provide initial support to families following a patient's passing.
  • Participate in bereavement assessments to ensure appropriate support for families.
  • Respect diverse cultural beliefs and ensure equitable care.
  • Contribute to health and safety initiatives and risk assessments.
  • Support junior staff, students, and volunteers in patient care.
  • Manage resources efficiently and maintain adequate supplies.
  • Act as a link nurse to promote evidence-based clinical practice.
  • Collaborate with external healthcare professionals involved in patient care.
  • Assist in developing and implementing policies to enhance care quality.

Experience and Skills

Essential:

  • Minimum of 2 years of postgraduate experience

Desirable:

  • Welsh language proficiency
  • Valid driver's license and ability to drive
