We are seeking a dedicated and compassionate individual to join our team as a Housing Support Worker in Bristol. As a key member of our team, you will work with vulnerable individuals to help them overcome challenges and achieve their goals.
Key Responsibilities:
- Progress and track referrals from the Local Authority into the service and outcomes for clients moving on from the service.
- Deliver a high-quality off the street accommodation service for individuals rough sleeping within the Bristol area.
- Complete holistic assessments for clients leading to the development of support plans which meet individual client goals.
- Implement person-centred packages of support with clients.
- Provide advice, information, and an enabling service to help clients access appropriate services and develop independence.
- Offer an effective Housing Management service for clients and identified properties.
