Job Title: Community Connector

Shaw Trust is a dynamic organization that promotes inclusivity and team spirit. We are seeking a motivated individual to join our team as a Community Connector.

About the Role:

As a Community Connector, you will play a vital role in enabling people to take control of their health and wellbeing. You will work closely with participants, health professionals, and community organizations to signpost to existing activities and help develop new local provision to reduce social isolation and loneliness.

Key Responsibilities:

  • Facilitate and engage with participants to support their health and wellbeing goals
  • Work collaboratively with health professionals and community organizations to develop and deliver effective signposting and support services
  • Contribute to the development of new local provision to address social isolation and loneliness
  • Evidence the outcomes of the service and contribute to the evaluation of its effectiveness

Requirements:

  • Minimum of SVQ/NVQ level 3 or equivalent
  • Experience of working with primary or secondary care services
  • Ability to facilitate, engage, motivate, and support participants
  • Highly organized with good planning skills and ability to deliver to agreed targets
  • Knowledge of the health and social care system and the application of wellbeing and prevention
