Key Responsibilities- Contribute to the ongoing assessment, planning, and delivery of patient care in line with our service pathways and clinical guidelines.
- Work collaboratively with the multidisciplinary team to ensure seamless care transitions and effective communication.
- Develop and implement individualized care plans that meet the unique needs of each patient.
- Provide education and support to patients and their families to promote independence and self-care.
- Participate in clinical supervision and peer review to ensure high-quality care and continuous improvement.
- Contribute to the development and implementation of clinical policies and procedures.
- Collaborate with other healthcare professionals to ensure effective care coordination and communication.
- Current registration with the Nursing and Midwifery Council (NMC).
- Experience working in a hospital or community healthcare setting.
