Your Responsibilities:- Collaborate closely with project teams and stakeholders to assess current practices regarding reasonable adjustments for cancer patients within our Trust.
- Develop and maintain a comprehensive resource database, guiding individuals through self-management tools and connecting them with relevant support organizations.
- Leverage exceptional communication skills to effectively engage with all levels of the cancer pathway, ensuring seamless coordination of care.
- Proactively identify and escalate any challenges or concerns related to reasonable adjustments to the project team.
- Create and manage a patient tracker to identify individuals affected by cancer who require reasonable adjustments, facilitating personalized support.
- Champion the Accessible Information Standard (AIS) within UHBW, including promoting its implementation across oncology services and educating staff involved in the cancer pathway.
- Participate actively in relevant forums and community engagement events to advance the adoption of the AIS within our Trust.
