About the Role

We are seeking a skilled and compassionate Rehabilitation Assistant to join our Home First & Reablement service in Cheltenham. As a key member of our team, you will play a vital role in supporting individuals to regain their independence and live safely in their own homes.

Key Responsibilities
  • Provide high-quality, person-centered care to service users, promoting their physical and emotional well-being.
  • Work collaboratively with the multidisciplinary team to develop and implement personalized care plans, focusing on reablement and rehabilitation.
  • Assist service users with daily living tasks, such as washing, dressing, and cooking, while maintaining high standards of hygiene and health and safety.
  • Monitor progress and function of service users, contributing to ongoing assessments and adjustments to care plans as needed.
  • Identify and escalate safeguarding concerns, ensuring timely and effective action is taken to protect vulnerable individuals.
  • Support service users and their families/carers in understanding and utilizing community resources, promoting digital inclusion and assisted technology.
Requirements
  • GCSE level Pass or equivalent in Maths and English.
  • Care Certificate.
  • Desirable: NVQ in a relevant topic (such as healthcare) or other relevant training.
  • Desirable: Experience of working with people, their families, and carers who may be going through challenging experiences or difficult situations.
  • Desirable: Experience of working in a health or social care environment providing personal and intimate care to people.
  • Desirable: Practical experience of motivating people, encouraging independence.
