Main Duties
  1. Triage incoming patient contacts via phone, email, or online platforms.
  2. Direct clinical queries to the Triage Clinician or other healthcare professionals, and administrative queries to the correct member of the team.
  3. Respond to patient queries via email or online platforms, within your scope of practice.
  4. Collaborate with the Pharmacists, Mental Health Worker, Social Prescriber, First Contact Practitioner, GPs, and nurses to ensure patients receive the most appropriate care.
  5. Work with the practice partners and manager to design and deliver service improvements to the online triage model.
  6. Contribute to service development through evaluation, audit, and system research.
  7. Maintain accurate, contemporaneous records of patient interactions, utilizing computer software systems and other mediums.
  8. Provide excellent communication skills as the first point of contact for the practice.
  9. Promote health education to our patient population, such as through participation in self-care initiatives.
  10. Support the practice in delivering on QOF, KPIs, and locally commissioned enhanced services.
  11. Update and maintain accurate patient medication records.
