About the Role

We are seeking a highly skilled and experienced Senior Case Manager to join our team at the Humber and North Yorkshire Mental Health, Learning Disability and Autism Collaborative.

The successful candidate will be responsible for providing expert clinical support to the Quality Assurance and Improvement Lead and Commissioning Leads in relation to the defined population.

This will include projected mapping of future service use, identification of users for future service provision, and planning in terms of future service needs.

Key Responsibilities
  • Manage individual patient pathways to ensure high-quality services and care planning outcomes are achieved.
  • Operationally oversee and support the delivery of day-to-day activities and projects.
  • Improve quality and outcomes through clinical assurance of the care provided by the relevant specialised services.
