Key Responsibilities

The successful candidate will:

  • Conduct Continuing Health Care Assessments and Reviews, employing a thorough, person-centered, and strengths-based methodology.
  • Collaborate effectively within a multi-disciplinary team to achieve optimal care outcomes.
  • Exhibit a solid understanding of the Continuing Health Care framework to inform practice and decision-making.
  • Perform Care Act Assessments and Reviews, adhering to the principles of the Act in all professional undertakings.
  • Present cases with clarity and confidence to relevant panels.
  • Participate in dispute resolution meetings and/or panel discussions as necessary.
  • Assess Mental Capacity and make Best Interest Decisions in accordance with legal and ethical standards.
  • Conduct Safeguarding Enquiries when required, prioritizing the safety and welfare of individuals.
Candidate Requirements

The ideal candidate will possess:

  • Registration with Social Work England
  • Eligibility to work in the UK
  • A relevant Social Work qualification (MA/BSc/DPSW/CQSW/MSc/BA)
