Registered Nurse for Primary Care

Folly Lane Medical Centre is a welcoming and dynamic healthcare facility dedicated to serving a diverse community. Our committed team strives to provide outstanding patient care, and we are currently seeking a Registered Nurse to join us for 20 hours per week, specifically on Mondays, Wednesdays, and Fridays. The successful candidate will play a vital role in delivering comprehensive healthcare services to our patient population. This position emphasizes evidence-based practices for managing long-term conditions and promoting preventive health measures.
Key Responsibilities
The primary responsibilities of the role include:
- Management of chronic health conditions
- Conducting assessments and developing care plans for patients with long-term conditions
- Ensuring safety protocols are followed
- Administering vaccinations and immunizations
- Providing support during emergencies
- Conducting both in-person and telephonic consultations as necessary
- Performing cervical smears and other routine procedures
- Covering treatment room duties, including hypertension management, phlebotomy, and ECGs

We are a forward-thinking training practice with a patient list exceeding 10,200 individuals. Our team is friendly and supportive, providing an excellent working environment. Our clinical staff includes:
- Multiple GP partners
- Salaried GPs
- Trainee GPs
- A dedicated nursing team, including Nurse Practitioners and Healthcare Assistants
- Administrative and reception staff
- Mental health professionals
- Additional support staff, including Clinical Pharmacists and Care Coordinators
Job Duties
- Assess and implement treatment plans that enhance health and well-being
- Collaborate with healthcare professionals to manage and treat chronic conditions
- Monitor and review patient medications for effectiveness
- Provide health education and support to patients
- Participate in vaccination programs for all age groups
- Conduct holistic assessments for various health needs
- Maintain compliance with infection control protocols
- Ensure confidentiality of patient information at all times
Qualifications and Experience
