About the Role

We are seeking a skilled and compassionate Healthcare Assistant to join our team at One Health Lewisham. As a key member of our clinical team, you will play a vital role in delivering high-quality patient care and supporting our healthcare services.

Key Responsibilities
  • Conduct health checks for patients with long-term conditions, including diabetes, asthma, COPD, and hypertension;
  • Support health promotion programmes and initiatives to improve patient outcomes;
  • Undertake wound care, dressings, and other clinical tasks as required;
  • Collaborate with the clinical team to manage chronic disease clinics and support patients with complex needs;
  • Administer immunisations and ensure accurate record-keeping;
  • Maintain a safe and clean clinical environment, adhering to our IPC policy;
  • Deliver opportunistic health promotion and education to patients and their families;
  • Support long-term condition reviews and management, including diabetes, hypertension, COPD, and asthma.
Requirements
  • Healthcare qualification or equivalent level (Care Certificate);
  • Relevant experience working in a primary care environment, preferably within the last 2 years;
  • Ability to take bloods and perform other clinical measurements required for long-term condition management;
  • Full, clean driving licence and access to a reliable vehicle.
