About the Role

We are seeking a skilled and compassionate Healthcare Assistant to join our team at One Health Lewisham. As a key member of our clinical team, you will play a vital role in delivering high-quality patient care and supporting our healthcare professionals in providing exceptional services.

Key Responsibilities
  • Conduct health checks for patients with long-term conditions, including diabetes, asthma, COPD, and hypertension;
  • Support health promotion programmes and initiatives to improve patient outcomes;
  • Undertake wound care, dressings, and other clinical tasks as required;
  • Collaborate with the One Health Lewisham Clinical team to manage chronic disease clinics and support patients with complex needs;
  • Administer immunisations and ensure clinical waste is removed from clinical areas in accordance with our IPC policy;
  • Deliver opportunistic health promotion and support long-term condition reviews;
  • Work closely with our healthcare professionals to provide comprehensive patient care and support.
Requirements
  • Healthcare qualification or equivalent level (Care Certificate);
  • Relevant experience working in a primary care environment in the last 2 years;
  • Ability to take bloods and perform measurements required for long-term conditions;
  • Full, clean driving licence and access to your own vehicle.
