About the Role
We are seeking a motivated and experienced Community Mental Health Practitioner to join our team at Essex Partnership University NHS Foundation Trust. As a key member of our Older Adult Admission Avoidance & Home Treatment Team, you will play a vital role in supporting older adults with functional mental health issues to meet increased needs and rising risk to support admission avoidance and provide home treatment with a Home First approach.
Key Responsibilities
- Work with a new and growing team, existing community services, and established Dementia Admission avoidance services across Mid and South Essex to provide comprehensive support to older adults.
- Develop and implement effective care plans to meet the needs of older adults with functional mental health issues.
- Collaborate with multidisciplinary teams to ensure seamless care coordination and effective communication.
- Provide education and training to staff and patients on mental health and well-being.
- Contribute to the development and implementation of policies and procedures to improve patient care and outcomes.
Person Specification
We are looking for a candidate with:
- Registered Mental Health Nurse or Occupational Therapist qualification.
- Additional professional qualifications in Older Adult Mental Health/Dementia or experience.
- Evidence of personal and professional development in contemporary mental health practice and dementia care.
- Good understanding of relevant legislation and guidelines, including the Mental Capacity Act and Care Act.
- Ability to work effectively in a fast-paced environment and prioritize workload and competing demands.
