Community Mental Health Support Specialist

We are seeking a dedicated and compassionate Support Worker to join our team in Kent and Medway NHS and Social Care Partnership Trust. As a Support Worker, you will play a vital role in supporting individuals with mental health needs, including those with dementia.
Your primary responsibility will be to provide support and assistance to registered staff in carrying out mental health needs assessments and interventions. This will involve working with individuals, their carers, and groups to promote service provision for older adults with mental health and dementia-related needs.
Main Responsibilities- Support initial assessments and post-diagnostic support for individuals diagnosed with dementia.
- Collaborate with registered staff to develop and implement individualized care plans.
- Provide emotional support and guidance to individuals and their carers.
