About the Role

The Senior Community Mental Health Practitioner will work as a core member of the Community Mental Health team to co-ordinate the care of a complex caseload of service users supervising a Band 5 Community Practitioner/Band 3 Community HCSW where appropriate.

Key Responsibilities
  • Formulate care and risk management plans in accordance with identified need and ensure that these are recovery focussed and developed in collaboration with the service user and where appropriate their relative and carer.
  • Work under the supervision of the Deputy Team Manager/Team Manager to ensure that care plans are implemented and reviewed regularly.
  • Develop and maintain effective relationships with service users, their relatives, and carers to ensure that their needs are met and that they are supported in their recovery.
  • Contribute to the development and implementation of team policies and procedures to ensure that they are aligned with the Trust's values and objectives.
  • Participate in the recruitment and selection process for new staff members to ensure that the team has the necessary skills and expertise to deliver high-quality care.
