Key Responsibilities:- Collaborate with Care Coordinators and Social Workers to provide comprehensive assessments and reports for young people.
- Develop and implement Pathway Planning, assessments, and interventions to support the mental health and wellbeing of Care Experienced young people.
- Act as a Lead Practitioner for Mental Health, disseminating best practices and engaging in team meetings to share knowledge and expertise.
- Establish and maintain collaborative relationships with allied health and social care practitioners, as well as partners in Housing, DWP, and Community organizations.
- Stay up-to-date with evidence-based practices and integrate models of best practice into daily service delivery.
- Participate in professional supervision and annual appraisal reviews, taking ownership of practice quality.
- Engage in clinical supervision and risk assessments, liaising with mental health services across different boroughs and areas.
- Contribute to team working, supporting colleagues, and participating in team meetings and development events.
- Identify opportunities for improving Council Services and propose innovative solutions.
- Understand and apply Council's duties and responsibilities for safeguarding children, young people, and adults.
Requirements:
A Social Work Qualification or equivalent is required for this role.
