About the Role

The Maximising Independence Service (MIS) Reablement program is an integral part of the Adult Social Care division, dedicated to early intervention, prevention, and enhancing individual autonomy and well-being. Our mission is to empower individuals to achieve maximum independence through tailored information, guidance, and support in setting and reaching short-term objectives.

Service Overview

The reablement initiative facilitates prompt hospital discharges, adhering to the principle of "Home First". The START Team comprises a diverse group of professionals, including Reablement Workers, Senior Reablement Workers, Reablement Managers, Reablement Coordinators, Senior Practitioner Occupational Therapists, Occupational Therapists, and Community Care Officers. Together, they assess and assist individuals in their homes to recover skills and regain independence following hospital stays due to illness or injury.

The Home First Response Service (HFRS) collaborates with external agencies, such as TuVida and AMG, alongside Community Care Officers, to support timely discharges and coordinate with our Discharge to Assess Teams.

Who We Are Looking For

We are seeking two dedicated Occupational Therapists who possess the necessary skills, experience, and values that align with our mission. Specifically, we require:

  • 1 Occupational Therapist for a 37-hour position within the HFRS team
  • 1 Occupational Therapist for a 30-hour position within the START Team
