Key Responsibilities- Assess and provide care to patients with various health conditions, utilizing supportive, preventative, therapeutic, palliative, and rehabilitative means.
- Collaborate with the site manager and other healthcare professionals to ensure seamless patient care.
- Develop and implement individualized care plans, taking into account patient needs and preferences.
- Conduct thorough assessments and interviews to identify patient needs and develop effective care plans.
- Work effectively in a team environment, contributing to continuous quality improvement and promoting a culture of patient safety.
- Participate in staff meetings and unit-specific projects, ensuring open communication and collaboration with colleagues.
- Adhere to Brightshores' 'CARE' Behaviours and 'LEAD' Competencies, promoting a safe and respectful work environment.
- Current registration with the College of Nurses of Ontario.
- Recent satisfactory performance and attendance record.
- Basic computer knowledge and proficiency in electronic medical records.
- Ability to work all shifts, including evenings, weekends, and holidays.
- Physical Assessment Course and Palliative Care Experience are assets.
- Two years recent medical/surgical experience.
