Job Title: Family Health Practitioner

We are seeking a highly skilled and dedicated Family Health Practitioner to join our team at the NHS. As a Family Health Practitioner, you will be responsible for providing high-quality patient care and working closely with our team to deliver excellent healthcare services.

Key Responsibilities:
  • Undertake a variety of duties including surgery face-to-face, telephone consultations, and where applicable, visiting patients at home.
  • Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other healthcare workers/organisations.
  • Assessing the healthcare needs of patients with undifferentiated and undiagnosed problems.
  • Screening patients for disease risk factors and early signs of illness in consultation with patients and in line with current practice disease management protocols and developing care plans for health.
  • Providing counselling and health education.
  • Compiling and issuing computer-generated acute and repeat prescriptions.
  • Completion of medical records.
  • Adhering to equality and diversity.
Person Specification:
  • GMC registration
  • Currently on the All Wales Performers List
  • Evidence of other specialist interest/development/training
Requirements:
  • Team skills- ability to communicate with, support and motivate
  • Self-motivated, flexible and can meet deadlines
  • Ability to prioritise work but aware of own limitations
  • Able to provide evidence-based solutions to problems/issues
  • Genuine commitment to improve quality in Primary Care
  • Excellent oral, written and interpersonal skills
  • Understanding of GMS contract
  • Initiative and Innovation
  • Welsh speaker
