Role Overview: The Healthcare Support Practitioner plays a vital role in the provision of comprehensive physical health assessments and care plans, working closely under the guidance of qualified practitioners.

Key Responsibilities:

  • Assist in the development and implementation of care plans aimed at enhancing the health, well-being, and social inclusion of service users.
  • Report any concerns or identified physical health issues to the multidisciplinary team, including nurse practitioners and general practitioners.
  • Facilitate effective communication among the community physical health team, service users, their families, and other healthcare providers.
  • Utilize a range of verbal and non-verbal communication techniques to address barriers faced by service users with learning disabilities and mental health challenges.

Safety and Environment:

As directed by qualified practitioners, contribute to maintaining a safe environment for service users, staff, and visitors.

Assessment and Intervention:

  • Carry out specific duties that aid in the assessment of service users' physical health and facilitate appropriate health interventions.
  • Engage in activities that enhance service users' functioning and promote meaningful engagement for individuals and groups.

Conflict De-escalation:

Employ skills and techniques acquired through mandatory training to effectively de-escalate situations involving physical or verbal aggression from service users.
