Job Summary: We are seeking a skilled and compassionate Social Care Professional to join our team at 5 Star Recruitment. As a Social Care Professional, you will play a vital role in supporting adults with recognized learning disabilities in the Rhondda Cynon Taff area.
About the Role: This is an exciting opportunity to work in a preventative, strength-based person-centred model under the principles of the Social Services and Wellbeing Act 2014. You will be part of a multi-agency team, working closely with colleagues from Health and other agencies to provide support and services to individuals with learning disabilities.
Key Responsibilities:
- Assess and develop support plans to ensure positive outcomes for individuals with learning disabilities.
- Work collaboratively with colleagues and agencies to provide a comprehensive support service.
- Practice under the principles of the Mental Capacity Act 2005, making applications to the Court of Protection as necessary.
- Contribute to the development of our work and influence policy and practice.
- Participate in regular supervision and peer supervision to ensure ongoing professional development.
Requirements: To be successful in this role, you will need to possess a professional recognized Social Work qualification and be registered and maintain your registration as a 'Social Worker' with Social Care Wales. You will also need to demonstrate strong assessment, communication, and planning skills, as well as an understanding of risk management and safeguarding.
