Join Our Team as a Social Worker

We are seeking a dedicated and compassionate Social Worker to join our team in Rhondda Cynon Taff. As a Social Worker, you will play a vital role in supporting adults with a recognised learning disability, working closely with colleagues from Health and other agencies.

About the Role

This is an exciting opportunity to work in a preventative, strength-based person-centred model under the principles of the Social Services and Wellbeing Act 2014. You will be expected to work collaboratively with colleagues to ensure positive outcomes for individuals with whom we work.

Key Responsibilities
  • Assess and develop support plans for individuals with a recognised learning disability
  • Work closely with colleagues from Health and other agencies to ensure effective multi-agency working
  • Practice under the principles of the Mental Capacity Act 2005, making applications to the Court of Protection as necessary
  • Develop and maintain strong assessment, communication, and planning skills to ensure positive outcomes for individuals
  • Contribute to the development of our work and be supported by a strong, experienced management team
