We are seeking a skilled Mental Health Practitioner to join our Community Mental Health Integrated Network Team (MINT) in Hounslow. As a key member of our team, you will play a vital role in providing high-quality support to individuals with mental health conditions in the community.

Key Responsibilities
  1. Work collaboratively with the local GP practices, primary care networks, social services, and other organizations to provide holistic support to individuals with mental health needs.
  2. Provide professional guidance and support to the new primary care network-based teams, focusing on mental and physical health support.
  3. Engage with local communities and help individuals access available resources and services.
  4. Support the development of person-centered care plans, working closely with individuals and their care teams.
  5. Contribute to the reduction of repeated assessments and storytelling, ensuring easier access to the right level of care.
