Key Responsibilities- Assess and develop treatment plans for individuals with significant mental health conditions.
- Provide evidence-based interventions and therapies to promote recovery and well-being.
- Collaborate with multidisciplinary teams to ensure seamless care coordination and delivery.
- Work closely with district nurses and therapy staff to provide comprehensive care.
- Clinical assessment and management of mental health needs.
- Relevant professional registration (Registered Mental Health Nurse, Occupational Therapist, or Social Worker).
- Training in supervision and appraisal.
- Post-qualifying training and experience in mental health.
- Significant experience working with older adults and complex mental health conditions.
- Ability to travel independently and work flexibly.
