About the Role:
We are seeking an experienced and qualified Social Worker to join our Transitions Team at Lancashire County Council. As a Social Worker, you will be responsible for planning with young people as they move from children to adult services, incorporating some Deprivation of Liberty Safeguards (DOLS) and Care Programme Approach (CoP) work.
Key Responsibilities:
- Plan with young people as they move from children to adult services
- Work with young people with learning disabilities, neurodivergence, long-term physical health conditions, sensory impairments, and those who are care experienced
- Empower young people to recognize and achieve their aspirations and desired outcomes
- Work alongside families and professionals to promote increased choice and control over their lives
- Support young people to develop improved connectivity with their local community as they approach and enter adulthood
Requirements:
- Social Work England registered
- Social Work Degree or equivalent
- Enhanced DBS
