We are seeking a highly skilled and dedicated Mental Health Practitioner to join our Primary Care Mental Health Services team. As a key member of our multidisciplinary team, you will be responsible for delivering high-quality mental health care to individuals in a primary care setting.
Key Responsibilities- Assess and manage the mental health needs of clients in a primary care setting
- Develop and implement effective treatment plans to promote mental well-being
- Collaborate with GPs and other primary care providers to ensure seamless care coordination
- Provide education and support to clients, families, and carers to promote mental health awareness and understanding
- Contribute to the development and implementation of mental health policies and procedures
- Registered Nurse (Mental Health), Social Worker, or Occupational Therapist qualification
- Post-registration continuing professional development activity
- Experience of working in a primary care or community mental health setting
- Ability to work effectively in a multidisciplinary team
- Strong communication and interpersonal skills
