Job Overview

This position is within the Crewe Care Community, focusing on the assessment, management, and treatment of a diverse caseload of clients with intricate and/or multiple needs. This is a permanent, static position.

Main Responsibilities

We are seeking a dedicated, adaptable, and motivated Speech and Language Therapist to join our supportive and dynamic team at Crewe Care Community/CCICP.

Our services include both urgent and planned responses, facilitating early hospital discharges and aiming to minimize hospital admissions. We also provide support for discharge-to-assess beds.

The role encompasses a varied caseload, requiring demonstrated knowledge, experience, and relevant clinical skills in managing patients with complex and long-term conditions, including falls, orthopaedics, general mobility issues, and neurological disorders.

The successful candidate will be an integral member of our multidisciplinary team, collaborating with District Nurses, Advanced Nurse Practitioners, Complex Case Practitioners, and MacMillan Nurses, while also supervising junior staff and students.

Administrative responsibilities are included, necessitating proficiency in computer use, particularly with Microsoft Office applications, and accurate typing skills. Our team leverages digital technology to facilitate remote working and accommodate flexible working arrangements, supported by access to a laptop and mobile device.

A valid driving license and access to a personal vehicle are required.

Clinical Practice

As a specialist health and care professional, you will be expected to:

  • Manage a clinical caseload autonomously, working without direct supervision.
