About the Role

We are seeking a highly skilled and experienced Community Mental Health Practitioner to join our Dementia Intensive Support Service team at Essex Partnership University NHS Foundation Trust.

Key Responsibilities
  • Provide high-quality, evidence-based care to patients with dementia and their families, working collaboratively with multidisciplinary teams to assess, plan, implement, and evaluate care plans.
  • Conduct comprehensive assessments, risk assessments, and care planning, ensuring that patients receive the most appropriate and effective care.
  • Work closely with the Advanced Nurse Practitioner to develop and implement care plans that meet the individual needs of patients.
  • Supervise and manage junior team members, providing guidance and support to ensure that they are equipped to provide high-quality care.
  • Collaborate with key partner organizations, service users, carers, clinicians, and other practitioners to deliver services that meet the needs of patients and their families.
Requirements
  • Registered Mental Health Nurse or Registered General Nurse with a good standard of written and spoken English.
  • Good standard of education and excellent IT skills, including proficiency in Word, Excel, PowerPoint, and Outlook.
  • Current UK driving license and access to a car for work purposes.
  • Experience in the performance management and supervision of lower-banded staff, as well as knowledge of crisis interventions and home treatment practices.
  • Ability to work independently and as part of a team, with excellent verbal and written communication skills and a zest for delivering high-quality patient care and service delivery improvements.
