Adult Mental Health Practitioner

We are seeking a highly skilled and dedicated Adult Mental Health Practitioner to join our team. As a key member of our multidisciplinary team, you will play a vital role in providing intensive home-based treatment to individuals in need of support.
Your primary responsibility will be to work closely with service users, their families, and other healthcare professionals to develop and implement personalized care plans. You will also be responsible for conducting ongoing assessments, implementing and evaluating care needs, and ensuring the effective transition of care across the acute pathway.
We are looking for an individual with exceptional communication skills, the ability to work autonomously, and a passion for promoting independence and recovery. If you are a registered nurse, social worker, or mental health practitioner with significant post-qualifying experience, we encourage you to apply.
Key Responsibilities- Provide intensive home-based treatment to service users, as an alternative to hospital admission.
- Conduct ongoing assessments, implementation, and evaluation of care needs.
- Work closely with inpatient units to ensure effective transition of care across the acute pathway.
- Liaise with Community Treatment Teams to ensure safe discharge of service users.
- Lead the care for a specified group of service users.
- Mentor, supervise, and teach qualified and unqualified staff, including students.

Person Specification- Registered Nurse, Social Worker, or Mental Health Practitioner.
- Significant post-qualifying experience for Band 6.
- Speciality-specific Level 3 learning.
- Knowledge of another language.
- Clinical supervision training.
