About the Role
We are seeking a motivated and enthusiastic individual to join our District Nursing Service in the Conwy locality. As a Healthcare Support Worker, you will play a vital role in supporting our team to deliver high-quality care to patients in their own homes.
Main Responsibilities
Direct Patient Care: You will work under the supervision of a trained nurse to provide care to patients in their own homes, following a plan of care implemented and reassessed by a registered nurse.
Organising Tasks: You will be responsible for prioritising tasks and organising your workload to ensure efficient use of resources.
Assistance and Support: You will provide assistance and support to patients in maintaining an acceptable quality of life, including helping with daily living tasks and promoting independence.
Documentation: You will maintain accurate and legible documentation in accordance with Health Board Policy, ensuring confidentiality at all times.
