Job Summary

We are seeking a highly skilled and compassionate Registered Mental Health Practitioner to join our First Episode Psychosis Service and developing At Risk Mental State service.

Main Responsibilities
  • Collaborate with service users, their carers, and system partners to deliver high-quality care.
  • Provide and oversee the management and care of service users.
  • Produce accurate and timely documentation and maintain records in line with professional codes and trust policies.
  • Conduct comprehensive assessments and formulate personalized care plans.
  • Assess risk and identify potential hazards in service users' environments.
  • Attend multidisciplinary team meetings and report incidents as required.
  • Act as a care coordinator and provide education and support to service users and their carers.
  • Develop service users' understanding of mental illness and provide training packages for staff.
