About the Role

We are seeking a highly skilled and motivated Practitioner Specialist to join our team. As a key member of our Adult Social Care team, you will be responsible for working with individuals with complex needs, providing person-centred support and guidance to help them achieve their goals and aspirations.

Key responsibilities will include conducting Adult Needs Assessments and Reviews, working with service users and their families to identify and address their needs, and developing and implementing support plans to promote independence and well-being.

As a Practitioner Specialist, you will be required to work in a fast-paced environment, managing a caseload of complex cases and prioritizing tasks to meet deadlines. You will also be expected to work collaboratively with other teams and agencies to ensure seamless service delivery.

About You

We are looking for an individual with a strong background in social care, preferably with a Social Work or Nursing qualification. You will need to be able to demonstrate a positive approach, self-motivation, and enthusiasm for working with vulnerable adults. Excellent communication and interpersonal skills are essential, as is the ability to work effectively in a team environment.

As a Practitioner Specialist, you will be required to have a strong understanding of the principles of person-centred care and be able to apply this in your daily work. You will also need to be able to think creatively and develop innovative solutions to complex problems.
