Job Overview

We are seeking a highly skilled and compassionate Senior Nurse Practitioner to join our Crisis Care Practitioner Nurse team in the Hammersmith and Fulham Crisis Assessment and Home Treatment Team (CAHTT) at West London NHS Trust.

Main Responsibilities
  • To conduct comprehensive biopsychosocial assessments for individuals referred to the service in a mental health crisis.
  • To provide assessment for gatekeeping for inpatient admissions and ensure compliance with clinical governance and quality improvement.
  • To work closely with other internal services and external agencies to provide a high-quality mental health service for patients under the care of CATT.
  • To be part of an MDT and actively participate in care planning and risk assessment/management.
  • To administer and monitor medication in line with NMC code of conduct and Medicines management training.
  • To work autonomously and as part of a team to provide home visits and support to junior colleagues and mentor student nurses.
Person Specification
  • Registered Professional (Mental Health Nurse) with relevant post-registration experience and evidence of post-registration further development.
  • Mentorship qualification and experience of mentoring students.
  • Ability to communicate in a second language and knowledge of RiO patient record system.
  • Full UK driving licence and willingness to drive pool cars.
  • Availability to work a shift rota covering 7 days a week 365 days a year.
