Key Responsibilities- Conduct comprehensive assessments to identify individuals' needs and develop tailored care plans.
- Provide practical support, equipment, and adaptations to help individuals maintain their independence and quality of life.
- Deliver evidence-based therapeutic interventions and rehabilitation programs to promote optimal outcomes.
- Collaborate with multidisciplinary teams and external partners to ensure coordinated and effective service delivery.
- Maintain accurate and up-to-date records and provide reports as required.
- A recognized degree or diploma in Occupational Therapy.
- Current registration with the Health and Care Professions Council (HCPC).
- Experience working within a social care setting, ideally supporting adults.
- Excellent communication and interpersonal skills to engage with service users, carers, and professionals.
- The ability to manage a caseload, prioritizing and organizing your work effectively.
