As a key member of our team, you will play a vital role in ensuring the safe and timely discharge of patients, working closely with our multidisciplinary team to achieve this goal.
Key Responsibilities- Actively participate in discharge planning, working closely with patients, relatives, and healthcare professionals to ensure a smooth transition from hospital to home.
- Communicate effectively with patients, relatives, and colleagues, demonstrating excellent written and verbal communication skills.
- Respond to telephone enquiries and relay messages accurately, courteously, and competently.
- Work effectively as part of a team, supporting senior colleagues to implement agreed discharge plans.
- Attend weekly Length of Stay meetings and contribute to the discussion on patient flow and discharge planning.
- Be responsible for maintaining accurate and up-to-date bed positions for the Trust, emailing the information as required.
- Assist the Patient Flow Team Leader with audits, data collection, and review of bed utilization, producing reports to demonstrate results.
The Clatterbridge Cancer Centre NHSFT is one of three specialist cancer centres in England, providing non-surgical cancer treatment to patients across Cheshire and Merseyside. Requirements- Administrative or clinical experience in a healthcare setting.
- Excellent communication and interpersonal skills.
