As a key member of our team, you will provide personalized support to individuals diagnosed with dementia and their carers, empowering them to maintain their independence and improve their overall well-being.

Key Responsibilities
  • Assist individuals with dementia and their carers in identifying their needs, providing information and guidance to promote independence and improve their sense of well-being.
  • Develop and maintain relationships with local contacts, including health and care professionals, to ensure seamless and consistent support for individuals affected by dementia.
  • Provide support face-to-face, by phone, and online, adapting your approach to meet the unique needs of each individual.
  • Collaborate with partner organizations to develop networks and pathways that empower individuals to make informed choices about their care and support.
Requirements
  • Ability to communicate effectively with individuals from diverse backgrounds and adapt your approach to ensure understanding at all levels.
  • Understanding of dementia and the needs of those living with dementia and their carers, with experience in supporting individuals with other diagnoses or support requirements.
  • Excellent networking and collaboration skills, with the ability to work effectively with professionals to achieve positive outcomes for individuals.
  • Good IT skills, with experience working with databases and virtual meeting software.
  • Ability to travel independently across the local area when required.
