We are seeking a highly skilled Palliative Care Nurse to join our team at University Hospitals Sussex NHS Foundation Trust. As a Palliative Care Nurse, you will play a vital role in providing holistic specialist clinical assessment, intervention, support, advice, and education to patients, carers with palliative and end of life care needs.
Key Responsibilities- Provide specialist palliative care to patients and their families, working closely with multidisciplinary teams to ensure delivery of high-quality individualized care.
- Collaborate with wards, clinical and multi-disciplinary teams to ensure delivery of high-quality individualized care for patients and their families requiring specialist palliative and End of Life care.
- Advise and support clinical teams in all aspects of end of life care, including DNACPR, Respect and ACP discussions.
- Participate in the nursing rota to provide a 7-day palliative care service between the hours of 9am - 5pm.
- RGN adult nursing qualification.
- Teaching/mentorship qualification.
- Experience at band 5 in clinical area caring for patients with end of life and palliative care needs.
- Experience in supporting patients and carers with complex physical and psychological needs.
- Ability to work within a team and to influence, support and motivate others.
- Experience in multidisciplinary working.
- Ability to deal with difficult/distressing situations with varying levels of complexity.
- Ability to assess, plan, evaluate and deliver care according to changing needs.
