About the Role

We are seeking an experienced and skilled Clinical Lead to join our Community Nursing team in Portsmouth. As a key member of our team, you will be responsible for leading and managing a team of nurses to deliver high-quality patient care in the community.

Key Responsibilities
  • Lead and manage a team of nurses to deliver patient care in the community
  • Develop and implement care plans to meet the needs of patients with complex health conditions
  • Work closely with other healthcare professionals to ensure seamless care transitions and coordination
  • Monitor and evaluate the effectiveness of care delivery and make recommendations for improvement
  • Collaborate with the multidisciplinary team to ensure patient-centered care and optimal health outcomes
Requirements
  • Registered Nurse with a degree in nursing or equivalent
  • Experience in community nursing and leadership roles
  • Strong communication and interpersonal skills
  • Ability to work autonomously and as part of a team
  • Knowledge of community health services and policies
