About the Role
We are seeking a highly skilled and dedicated Social Worker to join our team at Active Care Group. As a Social Worker, you will play a vital role in providing high-quality, individualized care to our patients with complex needs.
Key Responsibilities
- Manage a defined caseload of patients, ensuring their needs are met and their rights are protected.
- Develop and deliver evidence-based packages of care, including psychosocial interventions where necessary.
- Work in partnership with patients, carers, and other agencies to ensure seamless transitions and optimal outcomes.
- Conduct assessments, care planning, and record-keeping to ensure compliance with relevant policies and regulations.
- Liaise with multidisciplinary teams to ensure comprehensive care and support.

Person Specification
- Professional Qualification (Degree or equivalent) in Social Work.
- Professional Registration.
- Experience of working with patients with complex needs and their families.
- Knowledge of relevant legislation, including the Mental Capacity Act and Deprivation of Liberty Safeguards.
